Job summary

Lighthouse Behavioral Health Solutions (LBHS) in Lancaster, OH, is seeking a case management professional to assess client needs and implement comprehensive plans. The role reports to a Case Management Supervisor and focuses on coordinating SUD and mental health services to support recovery.

Qualifications

• 2+ years of relevant experience in case management or social services.
• BCI checks, corporate compliance, and drug screening are required.
• CDCA certification required; degree in a related field preferred.

Responsibilities

• Coordinate and provide safe, timely, effective, efficient, equitable, culturally sensitive, and client-centered care.
• For SUD clients: handle case assignments, assess barriers to recovery across life domains; draft service plan; review progress.
• For MH clients: coordinate treatment planning with CPST services; collaborate with clinicians on treatment plans.
• Adhere to confidentiality laws (HIPAA, 42 CFR Part 2) and Lighthouse protocols.
• Complete billing and documentation accurately and timely; see clients weekly as clinically appropriate.
