Job summary

Lighthouse Behavioral Health Solutions, LLC in Newark, OH seeks a dedicated Case Management/CPST support to assess client needs and develop comprehensive plans within a recovery-focused continuum.

The role collaborates with clinicians, ensures HIPAA and regulatory compliance, and supports seamless care coordination. Schedule Monday–Friday 8:30am–5:00pm with a competitive hourly range in a community-focused setting.

Qualifications

• Must pass BCI check, all Corporate Compliance checks, and employment drug screen.
• Knowledge of Job and Family Services procedures.
• 2 years relevant experience in case management, social services, or a related field.

Responsibilities

• Coordinates and provides safe, timely, effective, efficient, equitable, culturally sensitive, and client-centered care.
• For SUD Clients: handles case assignments, assesses barriers to recovery in various life domains, may draft service plans, and reviews progress.
• For MH Clients: coordinates treatment planning with CPST services and works with the clinical team on the treatment plan.
• Familiar with diagnostic assessments, reviews diagnoses, symptoms, risk, functioning, and identified needs (Z-Codes) to determine eligibility.
• Collaborates with clinical staff in development of treatment plans and follow-ups.
• Keeps HIPAA, 42 CFR Part 2, and Mental Health regulations on confidentiality.
• Completes billing and documentation accurately and timely; conducts weekly client visits as appropriate.

Education

CDCA certification

Bachelor's Degree in Social Work, Psychology, Counseling, or a directly related human service field

Job Summary: Provide support services and foster community by assessing the needs of their clients and developing and implementing a comprehensive plan to address those needs. This position reports to a Case Management Supervisor.

• Coordinatesandprovidessafe,timely,effective,efficient,equitable,culturallysensitive,andclient- centered care.

• ForMHClients:Handlescaseassignments,assessesbarrierstorecoveryinthevariouslifedomains, coordinatesmentalhealthtreatmentplanningincludinggoalsforCPSTservices,providesinput,and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan.
• Isfamiliarwiththediagnosticassessmentcompletedfortheclientsserved,whichincludesareviewof diagnosis, identification of current symptoms, risk, functioning,identifiedneeds in the client's life domains shown as Z-Codes todetermineeligibility for Case Management or CPST services and the level of care needed.
• Workscollaborativelyfollowingupwithobservationswiththeclinicalcounselor,psychiatrist,and/or nurse practitioner in the development of the treatment plan.
• AdherestoLighthouse'sprotocolforappropriate\"caremanagement\"activitiesaspartofthecontinuum ofservices to meet each client's defined needs and as defined in the treatment plan.
• Provides follow-up andappropriate referralsto community resources are essential. Collaboration and coordinationofservicestoensurecontinuityofcareandefficiencyofservicesacrosssystemsisvitalto early recovery.
• Provideslinkageandassistanceinaccessinginpatientormorerestrictivetreatment,asmedicallynecessary.
• Staysabreastofbestpracticesinmodalitiesofinterventiontomaintainhigh-qualitycaremanagement that is medically necessary to client specific treatment plan.
• Evaluatesdailylivingfunctioningandassistclientsincreatinggoalstoimprovedailyliving.
• Completesbillinganddocumentationaccuratelyinatimelymanner.
• Seesclientsweeklyorasclinicallyappropriate accordingto theclient’slevelofcare.
• EnsuresallcommunityactivitiescomplywithHIPAA,42CFRPart2,andMentalHealthlawsand regulations onconfidentiality of PHI.
• Keepsabreastofcommunityresourcesandmakesneededreferralstomeetclientneedsonatimelybasis.
• Coordinatesandcollaborateswithothercommunityagenciesregardingclient'streatmenttopromote positive treatmentoutcomes. Participate in community collaboration meetings.
