Job summary

Vital Health in Ohio is seeking a dedicated Case Manager to coordinate services for clients with substance use and co-occurring disorders. You will meet clients at their level of care, advocate for needed resources, and connect them with housing, employment, and medical services.

Under supervision, you will develop ISPs, collaborate with treatment teams, ensure timely documentation, and maintain a 20–30 client caseload while meeting weekly productivity.

Qualifications

• 3+ years of case management, peer support, or behavioral health experience.
• Associate or Bachelor's degree in Social Work, Psychology, Human Services, or related field preferred.
• Master's degree in related field is a plus.
• QBHS or QHMS certification preferred.
• CDCA certification preferred.
• Valid Driver's License and pre-employment drug screening required.

Responsibilities

• Provide community-based case management to clients with substance use and cooccurring mental health disorders.
• Assess clients' clinical and social needs, ensuring they receive appropriate care, services, and referrals.
• Develop, implement, and monitor ISPs in collaboration with the treatment team.
• Link clients to housing, employment, education, food assistance, medical care, and other essential services.
• Collaborate with psychiatrists, therapists, group facilitators, medical providers, and other professionals for holistic care.
• Assist clients in accessing MAT and other evidence-based interventions.
• Advocate for clients' rights within social services, legal systems, and medical institutions as needed.
• Maintain a caseload of 20-30 clients and document progress weekly.
