Lighthouse Behavioral Health Solutions, LLC is seeking a dedicated Case Manager to assess client needs, develop comprehensive plans, and coordinate care across SUD and MH services in Marion, Ohio. The role reports to a Case Management Supervisor and emphasizes timely, culturally sensitive, client-centered care.

Responsibilities include evaluating daily living functioning, providing referrals, documenting activities, and maintaining HIPAA compliance.

Qualifications

• CDCA certification is required.
• BCI check, all Corporate Compliance checks, and employment drug screen required.

Responsibilities

• Provide support services and foster community by assessing client needs and developing a comprehensive plan.
• Coordinate safe, timely, efficient, and equitable care; maintain client-centered focus.
• Collaborate with clinicians to develop and review treatment plans and assess barriers to recovery.
• Ensure confidentiality and HIPAA compliance in all communications and records.
• Complete billing and documentation accurately and timely; see clients as clinically appropriate.
• Engage in community collaboration meetings and referrals to ensure continuity of care.
