Qualifications

Responsibilities

• Facilitate client access to community resources and services.
• Coordinate and monitor services in relation to treatment plan.
• Assist in developing treatment plans and assess recovery status.
• Document all client encounters and billing documentation; maintain client files.
• Identify and provide emergency crisis services and crisis interventions.
• Assess clients and participate in treatment planning and case review.
• Maintain and report statistics regarding programs and client services.
