Job summary

Attain Behavioral Health is seeking a Housing Case Manager in Cleveland, OH to provide comprehensive, people-centered support to individuals in recovery, combining case management with hands-on guidance in the community.

You will help clients develop life skills, connect with housing, employment, and health resources, and advocate for self-sufficiency, while maintaining confidential documentation and collaborating with a multidisciplinary team.

Qualifications

• CDCA certification is required.
• QMHS III preferred.
• Minimum 2-3 years in behavioral health, substance use, or crisis response.
• Strong crisis management and de-escalation skills.
• Familiarity with Cleveland, Ohio area community resources.
• Valid driver’s license and reliable transportation.
• Maintain requested billing hours and professional boundaries.
• Empathy and professional boundaries in client work.
• Ability to work second shift as non-exempt staff.

Responsibilities

• Provide case management for clients in treatment, including assessment, care planning, and service coordination.
• Develop individualized service plans based on client needs, goals, and resources.
• Coordinate with clinical staff, peer support specialists, and external providers.
• Connect clients with housing, employment, mental health, medical, and legal resources.
• Monitor progress, track outcomes, and revise service plans as needed.
• Maintain timely, confidential documentation per policies and regulations.
• Advocate for clients' needs while promoting self-sufficiency.
• Participate in interdisciplinary team meetings, case reviews, and supervision.
• Stay current on local resources, behavioral health best practices, and regulatory requirements.
• Accompany clients in the community to practice skills (e.g., shopping, banking, job centers).
• Assist with life-skills development: budgeting, shopping, time management, job readiness.
