Job summary

Leora Behavioral Health posts a Case Manager, CDCA position in Ohio focused on helping clients access housing, food, clothing, school programs, vocational resources and mental health services to support recovery.

The role coordinates services, tracks treatment plans, and documents encounters for billing and records. Requires at least 1 year of related experience and Ohio CDCA certification, with eligibility to work at age 21 or older.

Responsibilities

• Facilitate client access to community resources, including housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services.
• Coordinate and monitor services, including comprehensive tracking of client activities in relation to treatment plan.
• Assist clients in developing goals and treatment plans and conduct medication and recovery status assessments.
• Document all client encounters and contracts; complete and submit billing documentation; maintain comprehensive client files.
• Identify and provide emergency crisis services; make clinical assessments and coordinate other services.
• Assess clients, evaluate treatment progress, and participate in planning and reviews with care providers.
• Maintain and report statistics regarding programs and client services.
• Be at least 1 year of experience directly related to the duties.

About Leora:

Leora Behavioral Health offers a full continuum of outpatient care to help support as many clients as possible. Their programs include a day treatment program (PHP), an intensive outpatient program (IOP), and general outpatient program. They also provide medication-assisted treatment (MAT), ambulatory detox, and case management services. Leora helps clients receive the support and treatment they need.

JOB RESPONSIBILITIES:

As Case Manager, CDCA, you will:

• Facilitate client access to community resources, including locating housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services; assist client to develop natural resources and make contact with social support networks.
• Coordinate and monitor services, including comprehensive tracking of client activities in relation to treatment plan.
• Assist clients in developing goals and areas of need and assist in developing treatment plans which are assessed regularly; conduct medication and recovery status assessment and determine required level and frequency of services.
• Document all client encounters and contracts made on behalf of clients; complete and submit billing documentation as appropriate; maintain comprehensive client files, which may include documents held for safekeeping on behalf of the client.
• Identify and provide emergency crisis services as necessary; make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques; coordinate other services as appropriate.
• Assess clients, evaluate effectiveness of treatment plan and progress made by client, and participate in client treatment planning and case review with client care providers.
• Maintain and report applicable statistics regarding programs and client services.
• At least 1 year of experience directly related to the duties and responsibilities specified.
• Must be certified as a CDCA (Chemical Dependency Counselor Assistant) in the State of Ohio
• Be 21 or older
