PurposeCare is seeking an experienced Community Care Program (CCP) Supervisor for our Oswego Private Pay team. This role centers on coordinating services, supporting caregivers, and ensuring high-quality home care aligned with CCP requirements.

Qualifications

• Previous experience within the Illinois Community Care Program (CCP) or an in-home care setting required.
• Strong understanding of home care operations and the needs of older adults and individuals with disabilities.
• Ability to lead, support, and communicate effectively with caregivers.

Responsibilities

• Oversee day-to-day coordination and delivery of CCP clients' services.
• Build strong relationships with clients, families, caregivers, and agency partners.
• Support caregivers and address scheduling, service, attendance, and performance concerns.
• Ensure clients receive consistent, dependable care according to service needs.
• Respond quickly and professionally to client or caregiver issues.
• Partner with agency leadership to resolve concerns and maintain client satisfaction.
• Assist with caregiver communication, coaching, and ongoing support.
• Maintain accurate documentation and help ensure compliance with program requirements.
• Identify opportunities to strengthen processes, caregiver retention, and client experience.
• Serve as a dependable resource for clients and caregivers while aiding Oswego team growth.
