Qualifications

• Previous experience within the Illinois CCP or in-home care setting is required.
• Strong understanding of home care operations and the needs of older adults and individuals with disabilities.
• Ability to lead, support, and communicate effectively with caregivers.
• Strong organizational, problem-solving, and conflict-resolution skills.
• Ability to prioritize multiple needs and remain responsive in a fast-paced environment.
• Excellent verbal and written communication skills.
• Client-focused mindset with professionalism and compassion.
• Ability to work independently while collaborating with agency leadership.
• Proficiency with Microsoft Office, including Word and Excel.
• Reliable transportation
• Legal authorization to work in Illinois

Responsibilities

• Oversee day-to-day coordination and delivery of CCP services for clients.
• Build strong relationships with clients, families, caregivers, and agency partners.
• Support caregivers and address scheduling, service, attendance, and performance concerns.
• Ensure clients receive consistent, dependable care according to service needs.
• Respond quickly and professionally to client or caregiver issues.
• Partner with agency leadership to resolve concerns and maintain client satisfaction.
• Assist with caregiver communication, coaching, and ongoing support.
• Maintain accurate documentation and ensure compliance with program and agency requirements.
• Identify opportunities to strengthen processes, caregiver retention, and client experience.
• Serve as a dependable resource for both clients and caregivers to support Oswego team growth.
