Job summary

RiverValley Behavioral Health seeks an Integrated Care Coordinator in Owensboro, KY to ensure access to coordinated, person-centered care. You will connect behavioral health, primary care, and social services, support timely referrals, and maintain up-to-date treatment plans.

You will collaborate with treatment teams, engage families, document progress, and uphold regulatory standards while supporting CQI efforts.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Sociology or related field.
• Prefer 2+ years in behavioral health or case management; training offered for new grads.
• Strong interpersonal and organizational abilities; proficient with EHR systems.

Responsibilities

• Facilitate linkage between behavioral health, medical and social service providers.
• Ensure timely referrals, follow-up appointments and feedback with the care team.
• Maintain current treatment plans and progress documentation.
• Advocate for the person’s needs, preferences and goals in care decisions.
• Engage families and support systems with informed participation in care planning.
• Coordinate across therapists, nursing, psychiatry, peer support and case management.
• Support warm handoffs between CCBHC services such as crisis, outpatient and MAT.
• Build collaborative relationships with local providers, agencies and referral sources.
• Identify community resources and address service gaps.
• Uphold CARF and CCBHC standards; document linking and coordination procedures.
• Participate in CQI initiatives and audits related to care coordination.
• Ensure compliance with Federal, State and local regulations and RiverValley policies.
• Complete mandatory HIPAA and regulatory training; maintain confidentiality.

Job Details: Job Location: USE FOR ALL OWENSBORO REQUISITIONS - Owensboro, KY, Position Type: Full Time, Education Level: Bachelor's degree, Salary Range: $19.23 - $21.63Hourly, Travel Percentage: Occasional, Job Shift: Day, The Integrated Care Coordinator serves as the primary liaison ensuring that each individual receiving CCBHC services has access to coordinated, comprehensive, person-centered care. This role integrates behavioral health, primary care, and social support systems to improve outcomes for individuals with behavioral and physical health needs. This position supports seamless communication and collaboration across internal programs and external providers and other services.

Essential Functions:

• Facilitate linkage between behavioral health, medical and social service providers in alignment with agency policies on service coordination.
• Ensure timely referrals, follow-up appointments and feedback loops among treatment team members and external agencies.
• Maintain comprehensive and up to date treatment plans and progress documentation.
• Serves as a primary advocate ensuring the person’s needs, preferences, and goals are all care decisions.
• Engage families and support systems as permitted and ensure informed participation in all care planning.
• Coordinate across all disciplines (therapists, nursing, psychiatry, peer support, case management) to ensure service delivery.
• Support warm handoffs between CCBHC services such as crisis, outpatient, MAT and community programs.
• Establish and maintain collaborative relationships with local healthcare providers, community agencies and referral sources.
• Assist in identifying community resources and addressing service gaps.
• Uphold CARF and CCBHC standards, ensuring all linking and coordination procedures are documented and compliant with organizational policies.
• Participate in CQI initiatives and audits related to coordination and service linkage.
• Ensures compliance with Federal, State, and local regulations and RiverValley’s mission policies, procedures and standards.
• Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.
• Maintains appropriate degree of confidentiality.

Qualifications: Education and Experience:

• Bachelor’s degree in Psychology, Social Work, Sociology or Behavioral Health-related discipline
• Prefer 2+ years of experience in behavioral health or case management, ideally within an integrated or CCBHC environment, but willing to provide training to new graduates.
