Job summary

RiverValley Behavioral Health, Inc. in Owensboro, KY is seeking an Integrated Care Coordinator to ensure access to coordinated, person-centered care across behavioral health, medical, and social services.

This full-time role coordinates referrals, maintains treatment plans, and advocates for patients’ needs, goals, and preferences while engaging families. On-site work with CARF/CCBHC compliance, CQI participation, and travel as needed.

Qualifications

• Bachelor’s degree required with relevant behavioral health focus.
• 2 years of experience preferred in behavioral health or case management.
• Training available for new graduates.

Responsibilities

• Facilitate linkage between behavioral health, medical and social service providers.
• Ensure timely referrals, follow-up appointments and feedback loops.
• Maintain comprehensive and up-to-date treatment plans and documentation.
• Advocate for the person’s needs, preferences, and goals in care decisions.
• Engage families and support systems in care planning.
• Coordinate across therapists, nursing, psychiatry, peer support, and case management.
• Support warm handoffs between CCBHC services.
• Establish and maintain relationships with local providers and referral sources.
• Identify community resources and address service gaps.
• Uphold CARF and CCBHC standards and ensure compliance.
• Participate in CQI initiatives and audits.
• Ensure compliance with regulations and RiverValley mission.
• Complete mandatory compliance and HIPAA training.

The Integrated Care Coordinator serves as the primary liaison ensuring that each individual receiving CCBHC services has access to coordinated, comprehensive, person-centered care. This role integrates behavioral health, primary care, and social support systems to improve outcomes for individuals with behavioral and physical health needs. This position supports seamless communication and collaboration across internal programs and external providers and other services.

Essential Functions:

• Facilitate linkage between behavioral health, medical and social service providers in alignment with agency policies on service coordination.

• Ensure timely referrals, follow-up appointments and feedback loops among treatment team members and external agencies.

• Maintain comprehensive and up to date treatment plans and progress documentation.

• Serves as a primary advocate ensuring the person’s needs, preferences, and goals are all care decisions.

• Engage families and support systems as permitted and ensure informed participation in all care planning.

• Coordinate across all disciplines (therapists, nursing, psychiatry, peer support, case management) to ensure service delivery.

• Support warm handoffs between CCBHC services such as crisis, outpatient, MAT and community programs.

• Establish and maintain collaborative relationships with local healthcare providers, community agencies and referral sources.

• Assist in identifying community resources and addressing service gaps.

• Uphold CARF and CCBHC standards, ensuring all linking and coordination procedures are documented and compliant with organizational policies.

• Participate in CQI initiatives and audits related to coordination and service linkage.

• Ensures compliance with Federal, State, and local regulations and RiverValley’s mission policies, procedures and standards.

• Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.

• Maintains appropriate degree of confidentiality.

Qualifications:Qualifications

• Education and Experience:

• Bachelor’s degree in Psychology, Social Work, Sociology or Behavioral Health-related discipline

• Prefer 2 years of experience in behavioral health or case management, ideally within an integrated or CCBHC environment, but willing to provide training to new graduates.

Knowledge and Skills:

• Strong interpersonal and organizational skills.

• Proficiency with EHR systems and documentation standards

• Knowledge of community health resources and behavioral health systems.
