Job summary

Pathways Inc. - Kentucky seeks a compassionate Care Coordinator to support individuals in our CCBHC program, coordinating services across physical and behavioral health and community resources to promote wellness and recovery.

You will work with consumers, providers, and partners to implement person-centered care plans and document activities in the EHR. Kentucky nursing license and an associate degree in nursing are required.

Qualifications

• Minimum Associate Degree or Diploma in Nursing required.
• Current Kentucky nursing license required.
• Experience working with and accessing community resources
• Proficiency in accurate and timely documentation within an electronic health record (EHR)
• Demonstrated ability to collaborate effectively with multidisciplinary teams
• Experience working with special populations served under CCBHC guidelines preferred
• Strong organizational skills and ability to manage multiple priorities simultaneously
• Experience in public health, care coordination, social services, or behavioral health settings preferred

Responsibilities

• Coordinate services for consumers across physical health, behavioral health, and community support systems
• Facilitate linkages to community resources, including: Social services, Housing assistance, Educational programs, Employment services, Healthcare providers, Other support services as needed
• Ensure smooth transitions of care between providers and levels of service
• Develop and maintain collaborative relationships with community partners and service providers
• Support consumers in achieving goals identified in their person-centered plan of care
• Document all care coordination activities accurately and timely within the electronic health record
• Monitor consumer progress and assist in addressing barriers to treatment and recovery
• Participate in multidisciplinary treatment team meetings and case consultations
• Maintain compliance with CCBHC standards, agency policies, and regulatory requirements
• Advocate for consumer needs while promoting independence, wellness, and recovery

About The Role

Pathways is seeking a compassionate, organized, and detail-oriented Care Coordinator to support individuals receiving services through our Certified Community Behavioral Health Clinic (CCBHC). The Care Coordinator plays a vital role in ensuring consumers receive seamless, integrated care by connecting them with behavioral health, physical health, and community-based resources that promote wellness, recovery, and overall quality of life. This position works collaboratively with consumers, providers, and community partners to coordinate services and support a person-centered plan of care.
