Job summary

Family & Children's Services in Tulsa, OK seeks a Care Coordinator to join its Certified Community Behavioral Health Clinic (CCBHC) team. The role coordinates care across health services, focusing on trauma-informed, recovery-oriented, and person-centered care for adults with complex needs.

• Driver License required for local travel.

Responsibilities

• Attend and participate in daily and treatment team meetings.
• Document client progress and maintain records.
• Coordinate care across health services and community resources.
• Provide individual, family, and group services as needed.
• Track admissions and discharges from hospitalizations.
• Assist with referrals, scheduling, and care planning.
