Commonwealth of Pennsylvania's Fayette County MH/ID Unit is seeking a County Caseworker 1 in Uniontown. This full-time civil service position supports individuals toward safer, more independent lives by linking them to resources and planning personalized services.

You will develop individualized service plans, coordinate with providers, guide person-centered discussions, monitor services, oversee health and safety, and help clients access benefits.

Qualifications

• Two years of experience as a County Social Services Aide 3 and two years of college coursework which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsabilités

• Plan Development: Create individualized service plans that reflect a person's needs and preferences
• Service Coordination: Identify and connect individuals to providers, community supports, and natural resources
• Person-Centered Support: Guide planning discussions that highlight personal choice and control
• Service Monitoring: Confirm that services match what is written in the individualized service plan
• Health and Safety Oversight: Observe and review the well-being of individuals receiving supports
• Resource Linking: Direct individuals to benefits, services, and helpful information
