The Commonwealth of Pennsylvania seeks a County Caseworker 1 for Fayette County Behavioral Health Administration. Join a dedicated team that guides individuals toward hope and independence, delivering meaningful community support.

Responsibilities include creating individualized service plans, coordinating with providers, monitoring service delivery, and promoting safety and person-centered care. Full-time, 8:30 AM–4:30 PM, Monday–Friday, Uniontown PA, with no telework option.

Formación

• Two years of experience as a County Social Services Aide 3 and two years of college coursework which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.

• Service Planning: Create individualized service plans that reflect each person's needs and goals
• Resource Coordination: Connect individuals with providers, community supports, and natural supports that fit their needs
• Monitoring Supports: Check that services are delivered properly and match what is outlined in the service plan
• Health And Safety: Promote well-being by addressing safety concerns and supporting healthy choices
• Person Centered Support: Encourage individuality, choice, and control in the lives of the people you assist
