Responsibilities

• Manage a caseload of participants and administer program policies.
• Conduct weekly meetings to review progress and ensure attendance at required programming.
• Perform risk/need assessments and create individualized treatment plans.
• Prepare discharge/continuation plans and forward documentation to supervising agencies.
• Document participant progress and ensure accuracy in the company database.
