Job summary

Salt Lake County is seeking a Human Services professional to provide information, assess needs, and coordinate services for older adults across the county. You will work with clients, families, and partners to plan and refer resources, ensuring safety and well-being.

The role emphasizes collaboration with agencies, ongoing assessments, and a hybrid telecommuting option to balance field work with office-based duties.

Qualifications

• Bachelor’s degree or equivalent combination of related education and experience.
• A valid Driver’s License accepted by the State of Utah is required at the time of hire.
• A background check through fingerprinting and RAP Back enrollment is required.

Responsibilities

• Provides information and support to clients and families seeking access to services.
• Responds to telephone, on-site visits, and walk-in inquiries regarding health and well-being.
• Performs comprehensive assessments and annual reassessments for older adults.
• Facilitates support and follow-up between clients and staff as needed.
• Develops relationships with community partners serving older adults.
• Coordinates with Adult Protective Services and other agencies to ensure safety.
