Job summary

Salt Lake County seeks a dedicated information and services specialist to assist older adults by assessing needs, coordinating services, and linking with community resources. The role emphasizes collaboration with internal teams and external partners to ensure safe, comprehensive support for clients.

The position requires a Bachelor’s degree in Human Services or related field, a Utah driver’s license, and successful background screening. A hybrid work arrangement and benefits package are offered.

Qualifications

• Bachelor’s degree from an accredited college or university in a Human Services field or closely related field; OR an equivalent combination of related education and experience.
• A valid Driver’s License issued by the State of Utah must be obtained within six months of hire to operate a County vehicle.
• Must pass a fingerprint background check and enrollment in RAP Back per County HR policy requirements.

Responsibilities

• Provide information and support to clients and families seeking access to services and link to internal/external resources.
• Respond daily to telephone, on-site visits, walk-in inquiries, and health/well-being concerns including emergencies.
• Perform comprehensive assessments of health, environment, and support status and reassess annually for eligibility and services.
• Facilitate follow-up between clients and staff as needed.
• Develop and maintain relationships with community partners providing services to older adults.
• Coordinate with Adult Protective Services and other agencies to ensure client safety.
