Responsabilidades

Position Overview

Utah Case Management is looking for a compassionate and dedicated Social Worker to join our team! The Social Work Case Manager coordinates services and support for elderly and mental health clients participating in Utah Medicaid’s New Choices Waiver (NCW) program. This role works collaboratively with clients, families, service providers, facilities, and other members of the care team to assess client needs, coordinate appropriate services, advocate for clients, and help ensure their services are meeting their needs.

This is a flexible hybrid position combining in-person client visits with remote documentation. The position offers a flexible work schedule depending on caseload and client needs.

Case Management Responsibilities

• Maintain monthly face-to-face contact with assigned clients.
• Visit clients in their homes, assisted living communities, or long-term care facilities.
