Job summary

Qualifications

• Bachelor’s degree in Human Services or related field or equivalent experience.
• Valid Utah Driver’s License required at hire; must obtain Utah driver's license within six months to operate a County vehicle.
• Background check through fingerprinting and RAP Back enrollment required.

Responsibilities

• Provide information and support to clients and families seeking access to services.
• Respond daily to calls, on-site visits, walk-in inquiries, and concerns related to health and well-being.
• Conduct comprehensive assessments of health, environment, and support status for older adults.
• Facilitate follow-up between clients and staff as needed.
• Develop and maintain relationships with community partners who provide services to older adults.
• Coordinate with Adult Protective Services, Law Enforcement, Fire Departments, and Mobile Crisis Outreach Teams to ensure safety.
