Case Planner

(FFT-CW Case Planner, I)

Family System Model: FFT-CW

Compensation: $23.00/hour, Full-time, Benefits, Remote Flexibility

Department: Family Services

Organization: The Salvation Army, Syracuse Area Services

Position Overview:

The Case Planner I will participate in training and provide specialized case management using the family systems-based FFT-CW model for a caseload of 10-12 families. The goal is to reduce risk and increase protective factors to prevent out-of-home placement of children.

Responsibilities:
  1. Provide comprehensive intake and assessments for assigned clients.
  2. Maintain regular contact with families, conducting weekly meetings (including at least one with children present), and sessions as per FFT-CW standards.
  3. Assist families in developing and implementing FFT-CW service plans and interventions, focusing on behavioral change.
  4. Participate in FFT-CW training, consultations, and case conferences; collaborate with supervisors; maintain documentation and outcome monitoring.
  5. Develop individualized service plans in collaboration with DSS and other providers, considering family strengths and cultural background.
  6. Provide casework, crisis intervention, brokerage, and advocacy services.
  7. Coordinate with other service providers and agencies.
  8. Refer families for auxiliary services as needed.
  9. Complete required documentation promptly, including assessments, case narratives, and reports.
  10. Maintain liaison activities with partners and provide case recommendations to relevant authorities.
  11. Demonstrate cultural competence and align with the organization's strategic objectives.
Qualifications:
  • Bachelor’s Degree in Social Work or related field.
  • 1-4 years of relevant experience (e.g., social worker, case worker, program coordinator).
Additional Requirements:
  • Valid New York State Driver’s License.
  • Access to private transportation during work hours.
  • Flexibility to work after 5 PM.
  • Ability to meet The Salvation Army’s Driver Program standards.
