Job summary

Southern Highlands Community Mental Health Center in Welch, WV seeks a Case Manager/Care Coordinator Level Two for school-based behavioral health support for children and adolescents and their families.

You will coordinate assessments, develop person- or family-centered treatment plans with licensed staff, connect families with services, and advocate to reduce barriers to care in school and community settings.

Qualifications

• Psychologist with a master's or doctoral degree from an accredited program.
• Licensed Social Worker.
• Licensed Registered Nurse; or
• Bachelor's or master's degree from an accredited college or university in a qualifying human services field.
• A satisfactory driving record and the ability to travel as required are also necessary for this position.

Responsibilities

• Provide school-based case management and care coordination to children, adolescents, and their families.
• Build positive, supportive relationships with students and families.
• Assist with assessments and person- or family-centered treatment planning in collaboration with licensed clinical staff.
• Identify behavioral, social, emotional, medical, educational, and other needs that may impact a student's well‑being.
• Connect students and families with behavioral health services, medical providers, social services, and community resources.
• Coordinate care with therapists, school personnel, healthcare providers, families, and community agencies.
• Provide advocacy and support for students and families navigating behavioral health and community service systems.
• Assist families with overcoming barriers that may prevent them from accessing needed treatment or resources.
• Conduct outreach and follow-up to promote engagement and continuity of care.
• Complete appropriate screenings, intakes, assessments, referrals, and documentation.
• Monitor progress and communicate with the treatment team to ensure services continue to meet the student's and family's needs.
• Participate in school and community activities that promote awareness of behavioral health services and available resources.

Case Manager/Care Coordinator Level Two (2) – School Based- McDowell County

Southern Highlands Community Mental Health Center is seeking a compassionate and motivated Case Manager/Care Coordinator Level Two (2) to provide school-based behavioral health support to children, adolescents, and their families.

The School-Based Case Manager/Care Coordinator works collaboratively with students, families, school personnel, behavioral health providers, and community agencies to identify needs, coordinate services, and reduce barriers to care. This position plays an important role in helping students and families connect with behavioral health treatment and other resources needed to support success at home, at school, and in the community.
