Position Summary/Responsibilities

Our Case Managers provide services to children, adolescents and young adults with developmental disabilities and their families. These services are designed to assist the family in accessing needed supports essential for living in the community and meeting basic needs, which includes medical, psychiatric, social, educational, vocational, and residential needs.

A Glimpse of Case Management

Case Managers are essential for ensuring that families receive person-centered services that help them live the life they want. A typical week usually consists of phone calls, meetings, collaborating with other agencies in the community, and monitoring progress which consists of home visits, attendance at school meetings, courts, and FAPT meetings. Case Managers' schedules are self-driven and subject to change at times; being organized and having the ability to adapt to changing situations are keys to success.

Hiring Range:

The anticipated base salary for this position is $45,178.91 to $48,184.72. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.

Requirements
  • Education: Bachelor's degree in a human services field. Required to receive continuing education annually in the amount specified by regulatory oversight bodies.
  • Experience:Clinical experience with children/adolescents is a plus.
  • Licenses/Certifications: Qualified Developmental Disabilities Professional (QDDP) a plus but not required.
  • Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
  • Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
Special Work Conditions

While performing the duties of this job, the employee will often sit, stand, walk, bend, stoop, crawl, climb and reach above their head. Use of hands for placing and pulling objects. The employee frequently lifts and or carries up to 25 pounds and pushes or pulls up to 150 pounds. Specific vision abilities required by this job include close vision (computer work). Specific hearing required by this job includes normal tones and clients with speech issues. Travel up to 15%. Travel is primarily local.
