Job summary

Oak Grove Recovery in Ohio seeks a Case Manager to coordinate inpatient and outpatient care within a multidisciplinary team for individuals with substance use disorders.

The role involves assessing biopsychosocial needs, arranging services, discharge planning, and maintaining documentation including FMLA and Short-Term Disability.

Required: Bachelor's degree or CADC/TCADC with a Bachelor's, valid driver's license, clean driving record, and commitment to a drug-free lifestyle.

Qualifications

• Bachelor's Degree or higher in a behavioral science preferred, or a CADC/TCADC with a Bachelor's Degree.
• Previous Targeted Case Management experience or supervision of the service since 2014.
• Valid, unrestricted driver's license.
• Clean driving record for at least 3 years.
• 1 year of experience working directly with adults or adolescents in a human service setting.
• Must be committed to a drug-free lifestyle and responsible alcohol use.
• Personal passion or interest in substance abuse issues strongly preferred.
• Must meet all requirements within State Regulations.

Responsibilities

• Assess patient out-of-facility biopsychosocial stressors (housing, finances, legal, medical, employment) and develop a strategy to meet these needs during and after treatment.
• Facilitate case coordination, information sharing, aftercare planning, and court-related issues.
• Maintain positive working relationships with support networks, government and community resources, employers, and referral sources.
• Complete all necessary documentation related to FMLA and Short-Term Disability.
• Maintain patient records by reviewing and completing case notes.
• Prepare patients for discharge by coordinating necessary community services to support sobriety post-treatment.
• Participate in weekly multidisciplinary treatment team meetings to assess patient needs and discharge planning.
• Participate in daily census management including coordination of discharge schedule and communication to the treatment and administrative teams.

Summary
