Qualifikationen

• Needs assessment with each participant at enrollment and ongoing updates.
• Develop and amend Individual Employment Plans within 10 business days of enrollment.
• Record keeping and documenting outcomes accurately in CEJA Reporting System.

Aufgaben

• Conduct enrollment needs assessments and update throughout the program.
• Create and monitor Individual Employment Plans for each participant.
• Meet participants weekly during training and address issues promptly.
• Recommend Barrier Reduction services and document justification.
• Connect participants with community resources and coordinate referrals.
• Collaborate with transition services for job readiness and placement.
• Maintain monthly follow-up for at least one year post-completion.
• Log services and outcomes in the CEJA Reporting System within five days.
