Job summary

Goddard Riverside Community Center in Manhattan is seeking a full-time case manager for Capitol Hall, a permanent supportive housing residence for formerly homeless adults. You will deliver client-centered services using Housing First, support entitlements, and coordinate with medical and social services to maintain residency.

The role requires a Bachelor’s degree in social work or related field, experience with mental illness or substance use, and strong documentation and communication skills.

  • Spanish speaking preferred.

Responsibilities

  • Provide client-centered case management using Housing First for formerly homeless tenants.
  • Maintain caseload, set short- and long-term goals, and document services and outcomes.
  • Assist clients with entitlements and understanding building rules/subsidies; coordinate with medical providers.
  • Coordinate group activities to reduce isolation and promote socialization.
  • Conduct room inspections, incident reporting, and crisis intervention as needed.

Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.Program Description: Capitol Hall is a permanent supportive housing residence for formerly homeless adults. On-site services include case management, entitlement assistance, medical care, substance abuse counseling, work readiness training, a meals program, 24 hour/7 day per week front desk security, and daily group activities.Position Summary: To deliver effective case management services using a strengths-based, client-centered approach to formerly homeless adults with a history of mental illness and/or substance use disorder, so tenants maintain permanent housing.Schedule: Monday through Friday 9:00am-5:00pmSalary Range: $46,000 - $55,000Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.Role, Responsibilities, and Essential Duties:1) Client serviceUsing the Housing First Evidence Based Model, provide client-centered case management services to caseload of formerly homeless tenants.Provide case management to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.Maintain case records with documentation of services provided and outcomes, according to specific program procedures.Assist clients in receiving entitlements and in understanding and complying with building rules and government subsidies to maintain residency.Coordinate and follow up with the onsite medical treatment providers.Accompany clients on visits to doctors, entitlement offices or other community services.Collaborate with staff in other GRCC programs to coordinate client service.Develop and facilitate group activities geared at increasing socialization and reducing isolation.Monitor and report all positive Covid cases and or symptoms and provide follow up as needed.Encourage participation among resistant clients; conduct floor meetings to introduce and encourage program participation2) Participate as Capitol Hall team memberAttend staff meetings, case conferences, professional development trainings and supervisory meetings.Participate in planning and leading group activities.Other duties as required include conducting room inspections, incident reporting and crisis intervention.Qualifications/Educational Requirements:Bachelor’s Degree in Social Work, Psychology, Counseling or related fieldProfessional or academic exposure to mental illness, substance use, and/or homelessnessSpanish speaking preferredSkills, Knowledge and Abilities:Experience with older adults; adults with mental illness and substance use disorder preferredUnderstanding of housing first modelExperience with harm reduction and motivational interviewing techniquesUnderstanding of trauma informed careAbility to multitask in a fast-paced environmentStrong interpersonal skillsTeam playerStrong verbal and written communication skillsComputer Skills to perform this job successfully, an individual should have/be:Proficient in Microsoft Outlook and ExcelAbility to use or learn computer databases such as AWARDSPhysical RequirementsSome walking and travel by public transportation to escort clients to various appointments throughout the city.Occasional visits to outside service providers, such as rehab centers, hospitals and medical clinics.Work EnvironmentMostly indoor office, occasional kitchen and basement setting during group activities and events.
