Job summary

Responsibilities

  • Provide supervision to case management staff and overall staff development.
  • Coordinate day-to-day program operations with the Program Director.
  • Interface with on-site medical and psychiatric teams and treatment providers.
  • Ensure timely documentation including progress notes and health forms.
  • Support evidence-based practices and data collection with a results focus.
  • Attend meetings, supervise audits, and participate in program improvement.

Job description

Program Description: Stephen Russo Residence (W 107th Street), is a permanent supportive housing residence for formerly homeless adults with mental health and co-occurring disorders. Services include case management, harm reduction, housing readiness, counseling, medical and psychiatric services, groups and recreation, and 24-hour 7 day a week front desk concierge.Position Summary: Provide support and clinical supervision to case managers and other social service staff with the goal of excellent service provision and helping clients to remain stably housed in a supportive environment while optimizing tenant health and well-being.Schedule: Sunday-Thursday 9:00am to 5:00pmSalary Range: $70,000-$75,000Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.Role, Responsibilities, and Essential DutiesEnsure provision of quality and timely services, including psychiatric and medical care, entitlements, assessment & treatment planning, and linkages to day programs, clubhouses, employment, and recovery. Act as Liaison with on-site medical and psychiatric team. Coordinate and manage day to day program operations along with Program Director.Supervision of case management staffEnsure that all documentation (including, but not limited to, progress notes, service plans, health forms, reports, etc.) is completed by direct service staff in a timely manner and within established deadlines as per agency and funding sources standards; provide coaching on policies and clinical practice.Support evidence-based practice models including person centered planning, motivational interviewing, trauma informed care, harm reduction, and strengths-based approachEnsure collection, tracking, and analysis of data, with a results-oriented mindset.Provide weekly supervision to staff; promote staff development, evaluate their work and initiate disciplinary action when required;Participate in new client interviews, intakes and admission meetingsCoordinate client care with all onsite treatment providers, serve as liaison with medical and psychiatric staffAssist with audit preparation and performance improvement plans as neededAssist Program Director with daily program operations, serve as senior leader in Program Director’s absenceInterface with various community agencies, funders and stakeholders relevant to our client populationCoordinate the scheduling of all contracted treatment providers and consultants.Collaborate with staff in other GRCC programs to coordinate client serviceWork with the Clinical Integration Team on the clinical best practices initiativeAttend staff meetings, case conferences, training programs and supervision meetingsParticipate in developing group curriculum and facilitate group activities as neededOther duties as requiredQualifications/ Educational RequirementsMaster’s Degree Preferred, Bachelor’s Degree considered with significant experienceSupervisory experience requiredExperience in supportive housing a strong plusExperience working with individuals diagnosed with mental illness, substance use and homelessness strongly preferred.Bilingual (English and Spanish) a plusSkills, Knowledge and AbilitiesStrong leadership skillsUnderstanding of homelessness, housing first, client centered approach, and harm reduction models preferredAbility to multi-task/work in a fast-paced environmentExcellent verbal and written communications skillsStrong interpersonal skillsAbility to work to collect, track, and analyze dataTeam orientedGood judgment and problem-solving skillsStrong organizational skillsComputer literateComputer Skills: To perform this job successfully, an individual should have/ be:Able to work in the Microsoft Office Suite, should especially be proficient in Word and OutlookAble to use or learn to use AWARDS databaseFamiliar with cloud-based payroll platforms such as Paycom (supervisor/manager functionality)Able to use or learn other electronic databases, programs, and funder-related software as neededPhysical RequirementsFrequent walking and standing, occasional liftingWork EnvironmentIndoor officeSome visits to client rooms requiredWe are committed to building an inclusive workplace community grounded in respect for differences.
