Job summary

The HOPE Program is seeking a Case Manager who will engage with training participants and alumni in both one-on-one as well as groups setting to provide counseling, guidance, and referral, and case management services to a case load of 120 to 150 individuals, including both low and high touch cases.

The position receives daily supervision from the Senior Case Manager and reports to the Director of Client Engagement.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or comparable background.
• Experience in case management or supportive services.
• Knowledge of trauma-informed care and NYC resources.

Responsibilities

• Provide counseling, guidance, and referral as part of case management.
• Maintain regular contact through meetings, calls, and visits to monitor progress.
• Travel weekly between program sites and community partner locations.
