Job summary

Claratel Behavioral Health in metropolitan Atlanta, Georgia, seeks a Case Manager (Paraprofessional) to provide direct client support across PES and CSU. You will assist with intake, triage, treatment coordination, and discharge planning, ensuring continuity of care for clients in crisis stabilization settings.

Responsibilities include daily assessments, updating treatment plans, group facilitation in CSU, and linking clients to community resources.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
• One year of experience in behavioral health, substance use disorder treatment, or crisis stabilization preferred.
• Master’s degree in Social Work (MSW) or Counseling preferred.

Responsibilities

• Intake & triage for PES and CSU clients.
• Daily assessments and updates to treatment plans.
• Provide 1:1 support and crisis de-escalation.
• Coordinate discharge planning and follow-up care.
• Maintain accurate documentation in the EHR and participate in daily treatment meetings.
