Breaking Ground operates nearly 4,000 units of housing across New York City, as well as in upstate New York and Connecticut. Our supportive housing combines affordable housing with services designed to help people maintain their homes long-term. This approach is recognized as a proven and cost-effective solution to chronic homelessness.

We go beyond providing housing. Our programs and services assist people experiencing street homelessness, especially those who have been on the streets the longest, to come indoors. Through our Street to Home outreach and transitional housing resources, we support individuals in moving towards permanent housing.

Located in Greenpoint, Brooklyn, the Continental Safe Haven offers 145 beds for individuals not yet ready to transition into permanent housing, or those who have struggled in other housing programs. It provides an alternative to street life: private, safe, clean, and affordable short-term accommodations. On-site case management and clinical services are available to meet residents' needs and facilitate their transition to secure permanent housing.

Reporting to the Clinical Coordinator, the Case Manager, Transitional Housing, will provide case management services to individuals experiencing homelessness, including those with mental illness and/or substance use disorders, to support their transition to permanent housing. This position has a 35-hour work week.

ESSENTIAL DUTIES:

  1. Provide direct services to a caseload of approximately 15 clients, including assessments, benefits assistance, and other support to achieve housing and recovery goals.
  2. Conduct psychosocial evaluations.
  3. Connect clients to psychiatric services.
  4. Link clients to medical and substance use treatment facilities as needed.
  5. Assist clients in obtaining identification, public benefits, and other income sources.
  6. Meet weekly with clients to assess needs, promote harm reduction, and develop service plans.
  7. Prepare documentation for housing applications.
  8. Assist and counsel clients in obtaining permanent housing.
  9. Maintain records in compliance with agency, city, and state standards.
  10. Intervene in crisis situations.
  11. Escort clients to appointments when necessary.
  12. Provide assistance with self-administration of medication after training, following agency policies.
  13. Perform other duties as assigned.

MINIMUM QUALIFICATIONS:

  1. Bachelor’s Degree or equivalent work experience.
  2. Two years of experience working with homeless adults and substance abuse issues (preferred).
  3. Experience with harm reduction and motivational interviewing techniques (strongly preferred).
  4. Valid driver’s license from NY, NJ, CT, or PA with a good driving record (preferred).
  5. Proficiency in Microsoft Word and Excel, with ability to learn new programs.
  6. Excellent writing and communication skills.
