AtlantiCare is hiring a Substance Use Case Manager for a role focused on comprehensive case management services for individuals with substance use disorders. This position entails assessing client needs, creating personalized service plans, and coordinating treatment with various stakeholders to ensure effective recovery.
Qualified candidates will possess a Bachelor's degree and preferred CADC certification. Responsibilities include conducting screenings, providing counseling, and facilitating referrals.

The Substance Use Case Manager will provide comprehensive case management services to individuals with substance use disorders, working evening expanded hours. The role focuses on assessing client needs, developing individualized service plans, coordinating treatment and support services, and promoting long‑term recovery. The case manager collaborates with the senior counselor, clients, families, treatment providers, and community resources to ensure continuity of care and uphold harm‑reduction, recovery‑oriented goals.
• Bachelor's degree in a related field required.
• CADC preferred; valid driver’s license required.
• Annual American Heart Association BLS and Heart Saver First Aid, and CPI certification required within 90 days of hire or transfer.
• Ability to pass a background check through fingerprint process and drug screen as required.
• Prior case‑management experience required.
The case manager will work closely with clinical staff, peer support specialists, healthcare providers, courts, probation, and community partners. They will participate in multidisciplinary team meetings and case conferencing, advocate for client needs across the system, and help reduce barriers to treatment.
This position requires work in office, community, and field settings, with occasional evening or weekend hours. The role involves repetitive motion, hearing, pushing, reaching, reading, speaking, standing, and walking, as well as use of a computer terminal, fax machine, copier, and telephone. Occasional driving is also required.
The position reports to department leadership.
