Job summary

The Institute for Family Health seeks experienced case managers to screen adults, children and families for insurance eligibility and benefit program enrollment. You will complete applications and provide intensive tracking, ensuring access to benefits and documentation is obtained.

Role requires collaboration with physicians and health care staff, thorough documentation in the EMR, and timely follow-up to optimize patient outcomes.

Qualifications

• Works collaboratively with physicians and other health care personnel to understand social and environmental factors affecting health problems.
• Provides direct case management services to assist patients in obtaining resources and services (Medicaid, Child Health Plus, Welfare, etc).
• Completes sliding scale, 340B pharmacy benefits, financial waiver, and cancer service applications as needed.
• Helps assist patients with insurance options including pooled trusts.
• Documents all patient services in the electronic medical record and closes encounters within 24 hours.

• Works collaboratively with physicians and other health care personnel in patient evaluations and treatment to further their understanding of significant social and environmental factors underlying patients' health problems.
• Provides direct case management services to assist patients in receiving necessary resources and services such as: Medicaid, Child Health Plus, Welfare, DAS, Social Security, Child Care, Home Care, Housing/Shelter/Safe Homes, Substance Abuse Treatment, Mental Health Treatment, HIV Services, Legal, other Health Insurance etc.

• Helps assist patients with insurance options including pooled trusts.
• Documents all patient services in patients electronic medical record and closes encounters within 24 hours.
