Job summary

Lifepoint Rehabilitation, part of Lifepoint Health, is seeking a Case Manager for Saint Francis Hospital in Colorado Springs. The role involves comprehensive patient assessments, coordinating care plans, and ensuring documentation meets medical necessity requirements.

We value RN licensure or equivalent healthcare licensure, CCM/ARN/ACM certifications, and at least 2 years of inpatient case management or social work experience. This is an on-site full-time position with competitive benefits.

Qualifications

• Current Registered Nurse or healthcare licensure as indicated (RN/PT/SLP/OT).
• Certification in Case Management or Rehabilitation Nursing recommended (CCM/ARN/ACM/ANCC RN-BC).
• Minimum 2 years inpatient social work or case management experience preferred; acute/rehab hospital experience valued.

Responsibilities

• Accurately perform patient assessments and identify patient needs.
• Identify and initiate appropriate nursing interventions.
• Provide care appropriate to condition and age of the patient.
• Perform timely and accurate documentation of medical necessity in the record.
• Complete and revise the Interdisciplinary Care Plan for each patient.
• Perform timely and accurate QI assessments.

How you'll contribute

ACase Manager who excels in this role:

What we're looking for

• We prefer a current Registered Nurse or Healthcare professional licensure as Physical Therapist, Speech Language Pathologist or Occupational Therapist
• We prefer a Certification in Case Management or Rehabilitation Nursing for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• Effective oral and written communication skills in English, additional languages preferred.

Hourlyrange: Salary range: $44-45 per hour.
