Job summary

The Salvation Army in Hamilton County seeks a dedicated practitioner to oversee social service programming, including Pathway of Hope and case management. You will manage client-centered approaches and coordinate referrals while maintaining documentation and reporting requirements.

Responsibilities include intake scheduling, community networking, and participation in divisional activities. Employment involves travel within Hamilton County and potential evenings/weekends as needed.

Qualifications

• Education: Associate degree in Human Services or related field.
• Background checks required and reviewed.
• Experience: at least one year of case management.
• Certifications: valid driver’s license; Safe From Harm training within 30 days; CCP within 30 days; SIMS within 30 days.

Responsibilities

• Schedule and conduct intake appointments for case management services.
• Implement strengths-based and client centered case management to identify goals and barriers.
• Apply Pathway of Hope Program model to support goals.
• Maintain referral sources for resources in Hamilton County and surroundings.
• Complete documentation in SIMS and other reporting per policies and funder standards.
• Network with other social services providers in Hamilton County.
• Represent The Salvation Army at public functions and EFSP Board meetings.
• Provide budgetary recommendations to leadership.
• Attend divisional training and identify community training opportunities.
• Participate in City Coordinating meetings as requested.
• Perform other tasks as assigned by leadership.
