Job summary

Qualifications

• Associate degree in Human Services or related field required.
• Background check required and reviewed.
• At least 1 year of case management experience.
• Valid driver’s license and Safe From Harm training within 30 days, CCP and SIMS training within 30 days.

Responsibilities

• Schedule and conduct intake appointments for case management.
• Apply strengths-based, client-centered case management to goals.
• Implement Pathway of Hope program model to support goals.
• Maintain referral sources across Hamilton County and nearby areas.
• Complete documentation for clients and resources in SIMS and reporting.
• Coordinate with other social services providers in the area.
• Represent The Salvation Army at public functions and on boards as requested.
• Make budgetary recommendations to leadership.
• Attend divisional training and participate in community meetings.
• Perform other tasks as assigned by leadership.
