Job summary

Odyssey House Louisiana, Inc. is seeking a full-time Case Manager to oversee eligibility for the Agency’s Low Barrier Shelter (LBS) and to develop effective individualized housing plans.

The role requires coordinating care, performing MI-based assessments, and connecting guests with community resources while maintaining accurate records and compliance with policies. A valid driver’s license and 2+ years of experience are preferred.

Qualifications

• Two years or more of case management experience.
• Excellent communication and organizational skills are required.
• Valid driver’s license is required.
• Experience with database management is preferred.
• Ability to work with homeless guests of diverse backgrounds and disabilities.

Responsibilities

• Provide case management to about thirty or more guests as assigned.
• Screen, assess, and develop individualized housing plans; monitor progress and link to community resources.
• Assist guests with obtaining vital records, IDs, licenses, and documentation.
• Assist with lease forms and obtaining supporting documentation.
• Document identification documents and other records as required.
• Assess Medicaid status and assist in maintaining eligibility per policy.
• Enter guest data into agency databases and keep records current.
• Prepare and submit reports to the Project Manager in a timely manner.
• Maintain occupancy and vacancy records; monitor daily activities in the shelter.
• Utilize Motivational Interviewing to guide behavior change and service planning.
• Refer guests to vocational, educational, housing, and social services.
• Help guests develop life skills to navigate the environment and access resources.
• Attend trainings and meetings; keep progress notes and referrals up to date.
• Ensure accuracy of assistance results and close files per policy.
• Provide crisis management when needed and complete incident reports.
