Qualifications

Responsibilities
Manage a caseload of up to 25 program participants.
Conduct interviews and administer assessments to understand clients' needs.
Create Individual Transition Plans for clients.
Establish and maintain relationships with partner organizations.
Monitor client progress and reassess needs regularly.
