Job summary

Community Care Alliance is seeking a Case Manager for the Safe Haven Outpatient program in Woonsocket, RI. The role focuses on care coordination and addressing social determinants of health for unhoused individuals with substance use and co-occurring mental health challenges.

You will work one-on-one with clients to remove barriers to care within a multidisciplinary team, including psychiatry, nursing, housing support, and peer recovery services.

Qualifications

• Associate degree in social services or Bachelor’s degree in social work, psychology, or related human services field, with at least one year of relevant experience.
• Must have a valid driver’s license, reliable transportation, and vehicle insurance.
• Bilingual applicants are strongly encouraged to apply.
