Job summary

Community Care Alliance is seeking a Case Manager for the Safe Haven Outpatient program in Woonsocket, RI. The role focuses on care coordination and addressing social determinants of health for unhoused individuals with substance use and co-occurring mental health challenges.

You will work one-on-one with clients to remove barriers to care within a multidisciplinary team, including psychiatry, nursing, housing support, and peer recovery services.

Qualifications

• Associate degree in social services or Bachelor’s degree in social work, psychology, or related human services field, with at least one year of relevant experience.
• Must have a valid driver’s license, reliable transportation, and vehicle insurance.

Case Manager- Safe Haven Outpatient (Full Time)

Location: Woonsocket, RI

Safe Haven outpatient offers a set of supportive services for individuals who are unhoused and actively using substances, often with co-occurring mental health challenges.

The Safe Haven outpatient team is a multidisciplinary team offering psychiatry, nursing, case management, peer recovery support, hospital liaison, and housing support.

The primary focus for this position is care coordination and addressing the social determinants of health with the intention of improving health and well-being. The case manager is integral to this goal, working one on one with clients to remove barriers to care.

The Support includes:

• Connection to emergency shelter and housing, access to basic needs,
• Primary care coordination,
• Access to benefits/entitlements, navigation of legal issues,
• Building improved natural supports and social environment.

Education/Licensing Requirements:

• Associate Degree in social Services or Bachelors degree in social work, psychology, or other related human services field, with at least one year of relevant experience.
• Must have a valid drivers license, reliable transportation, and vehicle insurance.
