Job Description

The Case Manager RN utilizes advanced nursing skills and knowledge to coordinate care for a defined patient population, facilitating patients through the continuum of care.

Qualifications For This Position Include
  • M-F 8:30-4:30
  • One weekend day shift every 4 weeks
  • One major holiday
  • Willingness to occasionally float to all 3 campuses
Required
  • BSN
Preferred
  • 2 years of medical-surgical nursing experience
  • 1-2 years of acute care, homecare, or population health RN case management experience
  • CCM certification
