Job Details

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.

Job Description

Case Manager RNs are responsible for the following:

1. Advocates for the patient and family throughout the entire episode of care.
2. Monitors patient’s progression and intervenes as needed to ensure efficient patient progress.
3. Coordinates all aspects of discharge planning and establishes a safe discharge plan.

Minimum Qualifications

• Bachelor of Nursing required
• Three (3) or more years of case management/discharge planning experience
• CCM certification preferred
• Active RN license in NJ/PA
