The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population to facilitate patients through the continuum of care. Responsibilities include coordinating all aspects of discharge planning and establishing a safe discharge plan for assigned patients.

Qualifications:

1. Education: Bachelor of Nursing required (Required)
2. Experience: Minimum of 3 years in case management or discharge planning (Required)
3. Certifications: CCM preferred; Active relevant Registered Nurse license in Pennsylvania or New Jersey (Required)
