Education and Experience:

1. Bachelors of Nursing required
2. Minimum of 3 years case management or discharge planning experience required

Minimum Certifications, Registration or License Requirements:

1. CCM preferred
2. Active relevant Registered Nurse license in Pennsylvania/New Jersey
