Job summary

Vibra Hospital of Central Dakotas in Mandan, ND is seeking a Case Manager to coordinate clinically complex patient care and discharge planning in a fast-paced LTAC setting.

Eligible clinicians include RN or Social Worker with BLS; LTAC experience preferred and CCM/ACM certification preferred. The role offers a $15,000 bonus opportunity and a comprehensive Vibra Healthcare benefits package.

Qualifications

• Current, valid, and active license to practice as a Registered Nurse, MSW or Licensed Social Worker in the state of employment required.
• Current BLS certification from a Vibra-approved vendor required.

Responsibilities

• Coordinate clinically complex patient’s care across a continuum and facilitate quality clinical and cost outcomes.
• Negotiate, procure and coordinate services and resources needed by patients; contribute to comprehensive discharge planning.
• Demonstrate knowledge to provide case management services to rehabilitation patients.
