Job summary

Applewood Centers is seeking a Case Manager & Respite Provider to deliver high-quality care coordination and short-term respite services to youth and families within the CTSOC system and OhioRISE program. The role uses the High-Fidelity Wraparound model to coordinate services across home and community settings, with a split caseload.

Qualifications

• Minimum High School Diploma required; Bachelor’s Degree preferred.
• QMHS certification required; LSW license preferred in Ohio.
• Core competencies include independent judgment, cultural competency, and professional conduct.
• Experience in systemic family therapy, crisis intervention, and family education preferred.

Responsibilities

• Provide Wraparound Care Coordination using the High-Fidelity Wraparound model for CTSOC clients.
• Complete assessments and maintain accurate clinical records per standards and regulations.
• Collaborate with formal and informal partners to coordinate or develop services.
• Deliver Therapeutic Behavior Services (TBS) and CPST where appropriate on assigned cases.
• Provide short-term respite care and supervision for youth aged 8–17, with transportation as needed.
• Engage youth with age-appropriate activities and monitor progress with treatment goals.
• Maintain open communication with youth, caregivers, and treatment teams.
• Uphold HIPAA and confidentiality across all client documentation.
