Qualifications

• High School Diploma required; Bachelor's preferred in Social Work, Counseling or related field.
• QMHS certification required; Ohio LSW license preferred.
• Experience in systemic family therapy, crisis intervention and family education; with diverse settings.
• Must have and maintain a valid driver's license and driving record suitable for transporting clients.
• Ability to work afternoon/evening/weekend hours and collaborate with a multi-disciplinary team.

Responsibilities

• Provide Wraparound Care Coordination using High-Fidelity Wraparound model for CTSOC-enrolled clients and families.
• Complete assessments and maintain accurate clinical records per standards and regulations.
• Coordinate with formal and informal partners; act as liaison to develop new services.
• Provide Therapeutic Behavior Services (TBS) and CPST where appropriate on assigned cases; participate in crisis management.
• Deliver short-term respite care for youth aged 8–17, including transportation to activities.
• Engage youths with age-appropriate activities and maintain clear communication with caregivers and treatment team.
• Maintain confidentiality and HIPAA compliance in all documentation and communications.
