Job summary

Applewood Centers in Cleveland, OH is seeking a Case Manager & Respite Provider to coordinate wraparound care and provide short-term respite for youth in home and community settings.

Under supervision of the Clinical Supervisor, you will manage assessments, collaborate with families and partners, deliver TBS/CPST services when appropriate, and ensure HIPAA-compliant documentation while supporting daily schedules including afternoons, evenings and weekends.

Qualifications

• Minimum High School Diploma required.
• Bachelor’s Degree in Social Work, Counseling or related field preferred.
• QMHS certification required. Valid Ohio LSW license preferred.

Responsibilities

• Provide Wraparound Care Coordination services using the High-Fidelity Wraparound model.
• Complete all assessments and maintain clinical records per standards and regulations.
• Collaborate with formal and informal partners for client services.
• Provide Therapeutic Behavior Services (TBS) and CPST when appropriate on cases.
• Deliver short-term respite care and supervision to youth, with transportation as needed.

QUALIFICATIONS:

Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.

JOB SUMMARY:

The Case Manager & Respite Provider, under the administrative/clinical supervision of the Clinical Supervisor of Foster Care/System of Care, is responsible for providing high-quality care coordination and direct behavioral health respite services to clients and families identified by the Cuyahoga Tapestry System of Care (CTSOC) or enrolled in the OhioRISE program, respectively. This dual-role position will have a split caseload coordinating formal and informal services using the High-Fidelity Wraparound model while also delivering direct, short-term respite care to support youth in a structured, safe environment across home and community settings. Work hours include afternoon hours with required evening and/or weekend availability to accommodate respite needs and family schedules.

ESSENTIAL DUTIES:

1. Provide Wraparound Care Coordination services as part of the CTSOC, utilizing the High-Fidelity Wraparound model to clients and families identified for the project across home and community settings.
2. Complete all required assessments and documents as outlined by the agency while maintaining accurate, timely clinical records in accordance with agency quality standards, CTSOC standards, and applicable licensing/accreditation regulations.
3. Work collaboratively with identified formal and informal partners on behalf of the Child and Family Team. Act as a liaison when new services or resources need to be coordinated or developed.
4. Provide Therapeutic Behavior Services (TBS), and Community Psychiatric Support Treatment (CPST) services where appropriate on assigned cases and participate in crisis management as necessary, to meet agency productivity standards.
5. Provide short-term, direct behavioral health respite care and supervision to youth (ages 8–17) in a safe home or in the community to offer caregivers planned relief. Provide transportation for youth enrolled in respite services to community activities.
6. Engage youth through companionship, structured recreational, educational, and therapeutic activities suited to their individual age, abilities, and treatment or care plan goals.
7. Maintain open, effective communication with youth, caregivers, and treatment team members, providing timely updates regarding activities, progress, and behavioral observations.
8. Respect client privacy and strictly adhere to confidentiality guidelines and HIPAA standards regarding all client-related documentation.

OTHER DUTIES:

1. Attend scheduled staff meetings, clinical supervision, and ongoing training as required.
2. Maintain all required trainings, certifications, and licenses per regulatory guidelines.
3. Maintain high standards of ethical and professional conduct and adhere to all agency policies and procedures.
4. Other duties as assigned by management.
