Job summary

The Center at Grande is seeking a dedicated Case Manager to join our skilled nursing team. You will track patient case information, coordinate plans, and support families throughout recovery and discharge planning to ensure smooth transitions.

• Minimum of 1 year Case Management experience preferred

What You'll Do
