Job summary

The Center at Arrowhead seeks a dedicated Case Manager to join its patient care team on site. You will plan, develop, and coordinate care in line with federal, state, and local standards to ensure the highest quality of care and smooth transitions from admission to discharge.

Collaborating with administration, medical and nursing staff, and community agencies, you will support discharge planning, document thoroughly, and uphold HIPAA compliance while guiding patients and families.

Qualifications

• Minimum one year case management experience (preferred).
• Must possess the ability to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to the Company.
• Must possess good judgement and have the ability to maintain highly confidential information.
• Must be able to pass a pre-employment background check/drug screen.

Responsibilities

• Meets with administration, medical and nursing staff to plan services for patients and families.
• Partners with patients, families, and care teams to ensure smooth transitions upon discharge.
• Ensures documentation is informative, descriptive, and follows policy and procedures.
• Assists with admission and discharge processes and participates in care plan meetings.
