Job summary

The Center at Grande in Tyler, TX is seeking a qualified Case Manager to join our skilled nursing team. You will track patient information, coordinate care, and support families through the treatment and discharge planning process.

Ideal candidates have strong communication, at least 1 year of case management experience, and a high school diploma. A background check and drug screen are required. We offer a comprehensive benefits package and a supportive, team-oriented environment.
